Care Coordination

Care Coordination at Sophia Recovery Centre

As part of Care Coordination, a woman will sit down with a Sophia Care Coordinator to discuss her unique circumstances, challenges, and preferences.

Our team helps a woman access both internal and external support services, creating a personalized recovery pathway tailored to each guest. Our Care Coordinator and Advocates check in regularly and remain connected with a woman for an extended length of time, ensuring ongoing support throughout her journey.

Parent-Child Assistance Program

PCAP is a three-year support program for women and birthing people who have used alcohol or drugs during pregnancy. Each participant is paired with a dedicated advocate who offers consistent, compassionate support to achieve their goals. It is trauma informed, recognizing that many moms have experienced deep trauma, often beginning in childhood.

Advocates respond with kindness and understanding. It is client led and every mom sets her own goals whether that’s recovery, housing, education, or parenting. Our advocates provide personalized, practical support every step of the way. Relationships are the heart of PCAP and advocates offer stability, mentorship, and trust—support that has often been missing for our guests.

Recovery Mentoring

Women can connect one-on-one with a Mentor through a twelve-week program structured around SMART Recovery tools. This is a program where a guest will set her own recovery-based goal and a mentor will facilitate her achievement of that goal with CBT tools that focus on 4 points: maintaining motivation, managing thoughts, feelings and behaviours, coping with urges and living a balanced lifestyle.

Other applicable resources are utilized such as journaling, mindfulness-based activities to build self-compassion. Our mentors are educators who are trained in SMART recovery, Motivational Interviewing, and more! They individualize the program based on their guests’ unique needs and goals.

We also offer a Family & Friends 12-week mentorship program tailored to support a person who has a loved one experiencing substance use health concerns. The program utilizes the same fundamental principles, the 4-points mentioned above and as well as CRAFT (community reinforcement and family training). Through one-to-one weekly support sessions, a guest is invited to focus on herself and her own care and goals, something that can often be forgotten when caring for a loved one.

Bridge to Recovery: Intensive Group Program

The Bridge to Recovery: Intensive Group Program is a comprehensive six-week outpatient therapy group designed specifically for women in early or precarious recovery from addiction. Grounded in Stephanie Covington’s acclaimed “Helping Women Recover” model and other evidence-based resources, the program is facilitated by a registered Social Worker and utilizes a trauma-informed approach.

The group explores recovery across four key areas: understanding the self, building healthy relationships, navigating sexuality and exploring spirituality. This holistic framework empowers participants to address the complex dimensions of recovery, fostering growth, resilience, and lasting change.
